  • : Sulfadoxine and pyrimethamine act by a sequential blockade in the biosynthesis of folic acid within the malarial parasite. Sulfadoxine has a structure similar to para-aminobenzoic acid(PABA) and blocks the conversion of PABA to the co-enzyme dihydrofolic acid , a reduced form of folic acid thtrathdrofolic acid is a co-enzyme that is important in the synthesis of pyrimethamine and purines which are essential in nucleic acid synthesis.

    Composition
    Each tablet contains Sulfadoxine 500mg and Pyrimethamine 25 mg.

    Indication
    treatment of plasmodium falciparum malaria especially in patients and in areas where P falciparum malaria to chloroquine is suspected.

    Contraindications

    Treatment of malaria if patients are hypersensitive to sulfonamides or pyrimethamine or the combination. It is also contraindicated in the first 2 months of pregnancy and in breast feeding mothers.

    Precautions

    Care should be exercised before prescribing this combination for patients with impaired renal or hepatic function, to those with a history of allergies or bronchial asthma.

    In glucose 6-phosphate deficient individuals haemolysis can occur due to sulfadoxine(as with some sulfonamides).

    Patients must be advised to take adequate fluids to prevent crystalluria and stone formation. Patients should be warned that sore throat, fever, pallor, jaundice, glottis and purpura may be early signs and symptom of serious disorders and the doctor should be informed immediately. If these were experienced during prolonged prophylaxis with it, periodic blood counts and analysis of urine for crystalluria may be necessary.

    Dosage and Administration
    Adults: 2-3 tabs. Children 9-14 years: 2tabs,4-8 years:1 tab,<4 years:1/2 tab All doses are to be taken 1  or 2 weeks before departure to an endemic area, administration should be continued during the stay and for 4-6 weeks after return.

    Adults : 1 tab once weekly or 2 tabs once every 2 weeks. Children 9-14 years:3/4 tab once weekly or 1/2 tab once every 2 weeks:4-8 years:1/2 tab once weekly or 1 tab once every 2 weeks.
